Understanding Fruzaqla (Fruquintinib) and Its Medical Uses

Fruzaqla (fruquintinib) is a selective inhibitor of all three VEGF receptor kinases (VEGFR-1, -2, and -3), developed by HUTCHMED and marketed by Takeda. Fruzaqla is the brand name used in the US and EU; the same active ingredient is marketed in China as Elunate.

Approved Indication

Fruzaqla is approved for adult patients with previously treated metastatic colorectal cancer, regardless of biomarker status, who have received prior fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, an anti-VEGF therapy, and (if RAS wild-type and medically appropriate) an anti-EGFR therapy. It was the first novel chemotherapy-free treatment approved for this patient population in the US in more than a decade.

How It Works

By selectively inhibiting all three VEGF receptor kinases, fruquintinib blocks angiogenesis (new blood vessel formation) that tumors rely on for growth and spread.

Dosing

The recommended dose is 5 mg orally once daily for the first 21 days of each 28-day cycle, taken with or without food at approximately the same time each day, until disease progression or unacceptable toxicity, as determined by the treating oncologist.

Warnings and Precautions for Fruzaqla

Fruzaqla requires monitoring for several adverse effects common to VEGF-pathway inhibitors.

Hypertension

Monitor blood pressure regularly and manage with antihypertensive therapy as needed.

Hepatotoxicity

Monitor liver function tests before starting and periodically during treatment.

Proteinuria

Monitor for signs of proteinuria during treatment.

Gastrointestinal Perforation and Fistula

Discontinue permanently if perforation or fistula occurs.

Hemorrhagic Events

Monitor for signs of bleeding; use caution in patients with a bleeding history.

Wound Healing Complications

Withhold Fruzaqla before elective surgery.

Embryo-Fetal Toxicity

Fruzaqla can cause fetal harm. Effective contraception is required during treatment and for a defined period after the final dose.

  • Regular blood pressure and liver function monitoring is required throughout treatment.
  • Report any signs of bleeding or abdominal pain to the treating physician promptly.
  • Discuss any planned surgery with your oncologist in advance.
