Imdelltra (tarlatamab-dlle) is a bispecific antibody that binds simultaneously to DLL3 (delta-like ligand 3), a protein expressed on the surface of most small cell lung cancer cells, and to CD3 on the surface of T cells, redirecting the patient's own T cells to recognize and destroy DLL3-expressing tumor cells. It is indicated for the treatment of:
Imdelltra received full traditional FDA approval in November 2025, after initially being granted accelerated approval in May 2024 based on response rate and duration of response; the confirmatory Phase 3 DeLLphi-304 trial supported conversion to full approval. Imdelltra is a prescription anticancer medicine and must be administered under the supervision of a qualified medical oncologist, in a setting equipped to recognize and manage CRS and neurologic toxicity.
Imdelltra is given as a fixed-dose intravenous infusion over 1 hour, following a step-up dosing schedule in Cycle 1 to reduce the risk of CRS:
Premedication with dexamethasone is given before each infusion in Cycle 1, along with post-infusion IV hydration on Days 1, 8, and 15. Patients are generally monitored in an appropriate healthcare setting for approximately 22–24 hours after the first two doses (Cycle 1, Day 1 and Day 8). They are advised to remain within about 1 hour of such a facility, accompanied by a caregiver, for 48 hours from the start of each of the first two infusions.
