What Is Orserdu Used For?

Orserdu (elacestrant) is a selective estrogen receptor degrader (SERD) that binds to estrogen receptor-alpha (ERα) to block its transcriptional activity and promote its degradation, including in cancers with ESR1 mutations that commonly drive resistance to earlier hormone therapy. It is indicated for the treatment of:

  • Postmenopausal women or adult men with ER-positive, HER2-negative, ESR1-mutated advanced or metastatic breast cancer
  • Disease progression following at least one line of endocrine therapy

ESR1 mutations are present in up to 40% of ER-positive, HER2-negative advanced or metastatic breast cancers. It is a prescription oncology medicine and must be used under the supervision of a qualified oncologist, typically after ESR1 mutation testing confirms eligibility.

Recommended Dosage

  • 345 mg (one tablet) taken orally once daily, with food
  • For moderate hepatic impairment: reduced to 258 mg once daily (three 86 mg tablets)
  • Not recommended for patients with severe hepatic impairment
  • Continued until disease progression or unacceptable toxicity

Orserdu Warnings and Precautions

  • Dyslipidemia: Elevated cholesterol and triglycerides are common (occurring in around 30% and 27% of patients respectively). Lipid levels should be monitored before and periodically during treatment.
  • Embryo-fetal toxicity: Orserdu can cause fetal harm. Effective contraception is required during treatment and for 1 week after the last dose, for both patients of reproductive potential and male patients with partners of reproductive potential.
  • Lactation: Breastfeeding is not recommended during treatment and for 1 week after the last dose.
