What Is Filspari Used For?

Filspari (sparsentan) is a dual endothelin and angiotensin II receptor antagonist. It is indicated for the treatment of:

  • Primary IgA nephropathy (IgAN) in adults at risk of rapid disease progression, generally with a urine protein-to-creatinine ratio of 1.5 g/g or higher

This indication is approved under accelerated approval based on reduction of proteinuria; the effect on kidney function decline has not yet been fully established. It is a prescription medicine and must be used under the supervision of a qualified nephrologist, with enrollment in the Filspari REMS program required before starting treatment.

Recommended Dosage

  • Starting dose: 200 mg once daily for 14 days
  • Then increased to a maintenance dose of 400 mg once daily, as tolerated
  • Taken before a meal, tablets swallowed whole with water

Filspari Warnings and Precautions

Boxed Warning — Hepatotoxicity and Embryo-Fetal Toxicity: Filspari can cause liver injury; liver enzymes and bilirubin should be checked before starting treatment and every 3 months during treatment. Filspari is contraindicated in pregnancy, as it may cause fetal harm. Because of these risks, Filspari is only available through a restricted program (the Filspari REMS), requiring patient, prescriber, and pharmacy enrollment and ongoing liver monitoring.

  • Hypotension: Including orthostatic hypotension (light-headedness on standing), has been reported.
  • Fluid retention: Swelling (peripheral edema) can occur.
  • Hyperkalemia: Elevated potassium levels should be monitored.
  • Acute kidney injury: Kidney function should be monitored during treatment.
