Voxzogo (vosoritide) is a C-type natriuretic peptide analog designed to address the underlying growth plate signaling defect in achondroplasia, the most common form of disproportionate short stature.
Voxzogo is FDA-approved to increase linear growth in pediatric patients with achondroplasia who have open growth plates (epiphyses), following a 2023 expansion that extended eligibility beyond the original 5-years-and-older population to children of all ages with open growth plates.
Vosoritide mimics C-type natriuretic peptide, counteracting the overactive FGFR3 signaling that suppresses bone growth in achondroplasia, thereby supporting more typical bone growth at the growth plate.
The recommended dose is 15 mcg/kg administered subcutaneously once daily. Patients should be well hydrated and have eaten before administration to reduce the risk of low blood pressure. Treatment is discontinued once growth plates close, as determined by the treating pediatric endocrinologist or geneticist.
Voxzogo requires careful administration technique and monitoring for the following considerations.
Symptomatic decreases in blood pressure (dizziness, fatigue, nausea) can occur, particularly in infants and young children. Patients should be well hydrated (approximately 240-300 mL of fluid within the hour before injection) and have adequate food intake before each dose.
Mild injection site reactions are common; rotate injection sites to reduce risk.
Regular monitoring of growth plate status is required to determine when treatment should be discontinued.
Blood pressure monitoring, especially during the initial treatment period, is recommended by the treating specialist.
